Html / Php form not adding to SQL database

Question

I've been working on this for hours now and including rebuilding my whole code I cannot figure out what is wrong. The HTML form is supposed to populate the patients SQL database but it is not working. I have a similar form that I am using on another part of the website that works flawlessly but this on doesn't seem to and a can't figure out why.

Are your $_POST even coming through? print_r($_POST); above your if test — clearshot66, Mar 30 '17 at 20:00

Then the `if()` statement at the beginning must not be succeeding. Check the values in `$_POST`. — Barmar, Mar 30 '17 at 20:04

That mean's the data is empty....like it should look like [name]=>Jeff[dob]=>03-30-2017 — clearshot66, Mar 30 '17 at 20:08

As answered on your duplicate post (also answering here for completeness), you're checking for a "doa" field in your if statement, which doesn't exist. Your form defines "dob" twice: once for Date of Birth, and once for Date of Admission:

<label for="dob">Date of Birth:</label>
<input type="text" name="dob">

...

<label for="doa">Date of Admission:</label>
<input type="text" name="dob">

this is the right answer as far as i can see, as he said you have no field for "doa" so it will always be "empty" and your if() statement will never execute. – ATechGuy Mar 30 '17 at 22:10

I am not completely sure about this being the safest way but instead of using MSQLI PDO why don't you try simple mysqli_query($conn,$sql);
