Android Call Display Designation

Question

In an Android mobile phone, when one recieves a call they can see a picture of the contact, name and mobile number. I want to know how to also show the Designation in that list too.

You can't modify the incoming call screen, only overlay it unless you are creating a custom Android ROM.
